- Completed Registration Form
- Copy of your current professional license
- Payment (we accept MC/VISA/Discover, checks, money orders, or electronic funds transfers)
NOTE: If you are registering for one of our advanced courses for Certified Lymphedema Therapists and you
completed your training with another school, please include a copy of your lymphedema therapy certificate.
